Health needs can become more complex with age. Next Practice Community can support older people, families and carers with general medical care, chronic disease management, preventive health, care coordination and discussions about changing needs.

Where appropriate, our team can work alongside families, carers, residential aged care teams and other health professionals to support coordinated, respectful care.
