A Heart Health Check is a dedicated appointment with your GP to help understand your risk of heart disease and plan practical steps to support your long-term health. At Next Practice Community, eligible patients can book a Heart Health Check to review key risk factors such as blood pressure, cholesterol, medical history, family history and lifestyle. Your GP will talk with you about your results and what they mean for your health.

A Heart Health Check may include:

  • A review of your medical history and family history

  • Discussion about smoking, alcohol, diet, movement, sleep and lifestyle
  • Diabetes and kidney health risk review where relevant
